Getting Started with eTEP Inguinal Hernia Repairs

Inguinal hernias are defined by the European Hernia Society as the protrusion of preperitoneal fat or abdominal viscera through a defect in the abdominal wall in the inguinal region [1]. The classical totally extraperitoneal (TEP) technique has limitations, including restricted space for dissection and mesh placement, limited port placement, intolerance to pneumoperitoneum, and challenges in teaching and learning the technique. These drawbacks may explain why the expertise in this technique is limited to fewer surgeons. Based on these considerations, an alternative extraperitoneal space along the anterior abdominal wall, with boundaries beyond the Retzius and Bogros spaces was developed, known as the enhanced or extended TEP approach (eTEP approach) [2].
