The options for minimally invasive complex ventral hernia repair have expanded with the advent of robotic surgery and advances in the field by Jorge Daes and Igor Belyansky. The totally extraperitoneal approach to hernia repair descriebd by Jorge Daes and expanded upon by Igor Belyansky to include the eTEP technique have allowed for the reapir of complex hernias without entering the peritoneal cavity. Here we describe the preoperative considerations, key anatomy, operative set up and the nuances between different eTEP approaches.
