Central chondrosarcoma (CSA) is a locally aggressive (low-grade) or malignant (intermediate to high grade) bone tumor producing a cartilaginous matrix uniformly and entirely chondroid in nature arising in the medulla of bone with diverse morphological features and clinical behavior. Grade 1 or atypical cartilage tumor (ACT) is low-grade, grade 2 is intermediate-grade, and grade 3 is high-grade malignant tumor. Primary central CSA arises de novo in normal bone without preexisting benign bone tumor (Figs. 21.1, 21.2, 21.3, 21.4, 21.5, 21.6, 21.7, 21.8, 21.9, 21.10, 21.11, 21.12, and 21.13). Secondary CSA may arise centrally in enchondroma(s) (EC) (Figs. 21.14 and 21.15) or peripherally in osteochondroma(s) (OC) (Figs. 21.16, 21.17, 21.18, and 21.19), especially multiple types of both entities. It can also develop in (polyostotic) fibrous dysplasia, synovial chondromatosis, Paget’s disease, and after irradiation.
