Low anterior resection (LAR) with TME total mesorectal excision remains the treatment of choice for tumors of the upper third of rectum. It may be performed upfront or after neoadjuvant chemo-radiotherapy. With gradual adoption of minimally invasive surgery, LAR is increasingly being performed using laparoscopic approach. This chapter outlines the indications, preoperative evaluation, technique, complications, results, and controversies associated with laparoscopic low anterior resection.
