Robotic Neck Dissection

Over many decades, head and neck cancer management has seen the evolution of various approaches for neck dissection, with surgical intervention being the mainstay for nodal disease management. Open neck dissection with its various incisions/approaches is still the gold standard and is primarily based on the location and extent of nodal disease. Technical advancement especially endoscopic and robotic-assisted procedures has now provided an alternate option of remote access approach to the neck, thus avoiding a discernible neck scar which is usually associated with neck dissection. Robotic neck dissection approaches have shown their feasibility along with adequate oncological outcomes. Cosmetic satisfaction is significantly higher in patients undergoing robotic neck dissection in comparison to the conventional approach. A few other advantages are improvement in neck movement, fibrosis, oedema and sensory loss. There is longer operative time and associated financial burden in patients undergoing robotic neck dissection which can be circumvented with experience and cheaper robotic platforms. Evolution and technical advancements are an ongoing process, leading to the development of more versatile robotic systems in the future, coupled with a surgical learning curve and experience that will further augment a wider application and acceptance of robotic neck dissection. The lack of objective data on various functional outcomes of robotic neck dissection warrants further studies.
