Traditional mail remains the main method of sharing electronic health record (EHR) data, mainly because there are no trustworthy and reputable methods for exchanging health data. Therefore, the patient's treatment is delayed significantly. By giving the patient priority within the system and enhancing the security and interoperability of health data, blockchain has the power to improve EHR. In order to save and exchange medical records, the Self-Serviced Medical Community (SMC) using the hyperledger fabric framework, delivers a private blockchain solution for EHR. Patients gain full control over who can see their medical records by utilizing grant and revoke access mechanisms. To create a safe medical community, the SMC-BC combines explainable artificial entelligence (XAI) and blockchain (BC). The existing system to manage ocular disease data are insecure and are not interoperable. Thus, SMC illustrates the benefits of utilizing XAI and BC technologies in electronic health record management (EHRM) with a specific use case of managing and predicting ocular diseases.
