Systemic control is critical to the curative treatment of localized sarcomas, in which distant metastases (as opposed to local recurrences) are both more common and deadly. Unfortunately, the vast majority of sarcomas do not demonstrate high sensitivity to systemic therapies. This tension between the importance of pursuing systemic control and the limited efficacy of available systemic agents represents the central challenge in the management of sarcomas.
