Leiomyosarcoma (LMS) is a malignant tumour of smooth muscle origin, most commonly arising in the retroperitoneum, extremities, and major blood vessels. This chapter reviews the clinical presentation, imaging features, pathology, and management of extrauterine LMS, illustrated by a case of retroperitoneal LMS arising from the inferior vena cava. Surgical resection is the primary treatment, though recurrence is common. Adjuvant therapies, including radiotherapy and systemic treatments, remain under investigation.
