Ptosis and diplopia are typical manifestations of OMG. These ocular muscles weakness has bilateral asymmetry, fluctuation, and shows the characteristics of being lighter in the morning and heavier in the evening, worsening after activity, and improving after rest or use of acetylcholinesterase inhibitors. The reasons why the NMJ of the extraocular muscles is more susceptible to impact than the NMJ of the skeletal muscles are as follows: (1) fewer synaptic folds, fewer postsynaptic AChR and Na+ channels, and a lower safety factor [1]; (2) susceptible to higher neuronal discharge frequency, prone to fatigue; (3) less expression of intrinsic complement regulatory factors, more susceptible to complement-mediated damage [2].
