The diagnosis of OMG is mainly based on: (1) Clinical manifestations: the clinical characteristics of fluctuating ocular muscle paralysis; (2) One of the serum AChR-Ab, MuSK-Ab, LRP4-Ab is positive; (3) Positive Edrophonium test or Neostigmine test; (4) Electrophysiological examination: positive repetitive nerve electrical stimulation decay or abnormal single fiber electromyography; (5) Effective treatment with acetylcholinesterase inhibitors. If the first criterion and any one of the other 2–5 are met, and other diseases causing ocular muscle paralysis are excluded, a clear diagnosis can be made.
