Intermediate nerve neuralgia (INN) is a very rare pathology. Its diagnosis is difficult due to intermediate nerve (IN) anatomy and functional features. INN surgical treatment is also challenging because there is no generally accepted surgical management protocol currently. The chapter describes IN anatomy, INN diagnosis, and treatment. Based on the author’s personal experience, an INN surgical treatment algorithm is proposed. Special attention is paid to intraoperative neurophysiological diagnostics.
