This chapter focuses on Transbronchial Needle Aspiration (TBNA) in Endobronchial Ultrasound (EBUS), a crucial technique for obtaining tissue samples from the lungs and mediastinum. It details the key considerations, types of needle used, including needle selection, technique optimization for both conventional and EBUS guided TBNA and the factors determining the outcome of cytological specimens. By providing practical insights and addressing potential challenges, this chapter serves as a valuable resource for clinicians seeking to master TBNA in the context of EBUS, ultimately improving diagnostic outcomes and patient care.Transbronchial needle aspiration (TBNA) is a bronchoscopy technique which is performed to obtain both histological or cytological sample specimen. Based on the type or site of aspiration required, needles of different sizes are passed either through the barrel of the rigid bronchoscope or through the working channel of the flexible bronchoscope. The tissues that can be obtained from various sites of lung through TBNA technique are like endobronchial tumours, lymph nodes, tissues adjacent to the airway wall and the peripheral lung nodules. The main purpose of transbronchial needle aspiration is to aid in the diagnosis and staging of bronchogenic carcinoma. Diagnosis of endobronchial tumours, submucosal lesions, externally compressing central airway tumours and the staging of mediastinal lymph nodes in bronchogenic carcinoma are all performed by transbronchial needle aspiration technique.
