A 36-year-old female presented with pain and blurry vision in the left eye which occurred suddenly upon awakening that morning. Her past ocular history was notable for a scratch by her daughter’s fingernail to the cornea of the same eye 2 years prior, followed by two similar episodes of eye pain upon awakening. Visual acuity was 20/20 in the right eye and 20/50 in the left eye. The left conjunctiva was diffusely injected, and there was an epithelial defect in the central cornea (Fig. 25.1). The rest of the examination was within normal limits. The patient was diagnosed with recurrent corneal erosion syndrome secondary to her history of corneal trauma. She requested that we prescribe a topical anesthetic, as she had been given one during an emergency room visit the previous year. We explained the risks associated with this practice and instead prescribed fluoroquinolone eye drops to be used four times a day in the left eye for a total of 5 days in addition to long-term use of lubricating agents. We explained the chronic course of the condition and that a stepwise management approach may be necessary to prevent recurrences.
