This chapter narrates the experience of managing a severe visual deficit due to atypical optic neuritis, in a middle-aged male who was a known case of keratoconus. The emphasis is on the importance of good clinical examination to elucidate the exact cause of the visual deficit. The chapter stresses the refined principles of diagnosis and management of a case of optic neuritis, especially with the advent of biomarkers.
