The Bumpy Road to Clear Vision: A Case of Unwanted Calcification of Hydrophilic Acrylic IOL Following DMEK Superimposed on Penetrating Keratoplasty

Failure of the corneal graft after penetrating keratoplasty is a known complication. Management includes replacing the graft with repeat keratoplasty. However, in cases of decompensation restricted to a particular layer of the cornea, even a partial lamellar keratoplasty of the graft can be done. Our chapter highlights the utility of performing DMEK in endothelial decompensation of a graft post-PK. This case however also revealed another complication of IOL calcification due to the use of intracameral SF6 gas as a tamponading agent. The difficult journey through the long period of dilemmas and challenges faced in this case is presented in detail for the readers with a thorough discussion on successful management in each scenario.
