Unilateral Vogt-Koyanagi-Harada Disease Associated with Non-arteritic Anterior Ischemic Optic Neuropathy in a Young Woman Presenting as Acute Angle-Closure Glaucoma

Vogt-Koyanagi-Harada disease (VKH) is a multisystem autoimmune inflammatory disorder affecting the eyes, inner ear, and skin. Association of anterior ischemic optic neuropathy (AION) with VKH is very rare, usually manifested in the fifth and sixth decades of life. Also, only a few reports have described a masquerade presentation of acute angle-closure glaucoma (ACG) as an initial manifestation of VKH, which is typically characterized by panuveitis with serous retinal detachments. This has mostly led to misdiagnosis, unwarranted surgical interventions, and a delay in appropriate, timely management. We describe an unusual case of unilateral VKH associated with non-arteritic anterior ischemic optic neuropathy (NAION) in a young woman presenting as acute ACG.
