For humans, the threat of disease is the never-ending problem in the lifespan of their living. In modern days, the treatments for these diseases with multiple new type symptoms and behaviors for each of these diseases have become more challenging for medical staff without proper information about the patients. The lack of information and records may lead to misdiagnosis or diagnostic error which will further cause death or cases of permanent disabilities (such as brain damage, blindness, loss of limbs or organs, and metastasized cancer). It is very important to address problems in the present system and enhance the system for better living. In this paper, we present a novel approach or a system to handle the medical information of a patient compiling to the regulation of the regulators and protecting the privacy of the patient. Our approach broadly has two main parts: (i) Collecting the data of the patient from various sources and (ii) Storing the data in a decentralized and secured way. These two processes enhance the quality and availability of the medical information to meet our goal.
