The delicate periorbital area presents a unique challenge in facial rejuvenation. The skin around the eyes is thin, sensitive, and constantly in motion, making it particularly prone to early signs of aging, including fine lines, wrinkles, and pigmentary irregularities. Among chemical peeling agents, phenol and trichloroacetic acid (TCA) remain among the most effective and widely utilized options in dermatosurgery. Owing to its ability to induce controlled epidermal and dermal injury followed by collagen remodeling, TCA peeling has long been employed for cutaneous rejuvenation. When appropriately performed, TCA peels can significantly improve skin texture, pigmentation, and fine rhytides, including in the challenging periorbital region. This chapter reviews the role of trichloroacetic acid (TCA) peeling in periorbital rejuvenation, with emphasis on patient selection, procedural technique, clinical outcomes, and potential complications. Practical aspects of TCA application in the periorbital region are discussed alongside considerations regarding safety, efficacy, and aesthetic outcomes. In addition, the authors share their clinical experience and approach to optimizing results while minimizing adverse effects.
