Managing Reproductive Issues in Systemic Lupus Erythematosus

Systemic lupus erythematosus (SLE) is an autoimmune disease that predominantly affects women of childbearing age. Despite significant advancements in the clinical management of SLE, pregnancy remains a considerable challenge for SLE patients, and may result in several adverse maternal and fetal outcomes, and disease flares. Well-thought-out strategies help in mitigating these issues. A comprehensive preconception evaluation is essential for optimizing outcomes. Patients should meet specific criteria before conception to ensure the best possible outcomes for both mother and fetus. During pregnancy, a multidisciplinary approach is crucial for coordinating care. This involves monitoring disease activity and fetal development through an individualized management plan. Prompt recognition and appropriate management of disease flares and pregnancy complications are vital for maintaining maternal and fetal health. Postpartum management is also important, focusing on addressing the risk of disease flares and supporting breastfeeding with safe medications. While guidelines exist, evidence gaps persist regarding the management of high-risk subgroups, the prevention and treatment of pregnancy complications, and the use of biologics during pregnancy and breastfeeding. Additionally, the underlying molecular mechanisms contributing to adverse outcomes in SLE patients remain poorly understood. Future research is necessary to further improve maternal and fetal outcomes in SLE.
