Managing Reproductive Issues in Rheumatoid Arthritis

The complex interaction between rheumatoid arthritis (RA) and pregnancy results in significant physiological and immunological changes that impact disease activity and pregnancy outcomes. During pregnancy, maternal tolerance to the semi-allogenic fetus is fostered through adaptive immune responses, which can influence disease process in RA. Several adverse pregnancy outcomes (APOs) such as preterm birth, pre-eclampsia, and low birth weight, have higher incidence in RA. Higher disease activity in RA is linked to APOs. Conversely, pregnancy can alter the course of RA, with some women experiencing temporary remission or symptom reduction due to hormonal and immunological changes. Understanding the immunological changes during pregnancy, including the balance between proinflammatory and anti-inflammatory responses, is essential for managing RA in pregnant women. Preconception planning and achieving low disease activity or remission are vital for improving fertility and pregnancy outcomes in women with RA. Overall, the multifaceted challenges of RA in pregnancy are best addressed through a multidisciplinary approach.
