Planning and Applying Symptom-Based Approach for Psychosis in Psychotic Disorders

This chapter provides the overarching framework for implementing a symptom-based CBT strategy in psychotic disorders. It begins with comprehensive assessment, then introduces three pillars for treatment planning: functional levels, psychotherapy requirements, and the combinability of therapeutic modules. The chapter outlines psychotherapy milestones, emphasizing stabilization of acute symptoms before higher-order cognitive work. It conceptualizes CBT for psychosis as a syndromic intervention, selecting modules such as insomnia treatment, anxiety management, or schema work based on the individual’s presenting problems. The chapter stresses that heterogeneity in psychosis necessitates flexible sequencing and prioritization, ensuring that each patient receives only the interventions that match their symptom profile and functional stage.
