Chemical Sterilization

This chapter provides a comprehensive overview of chemical sterilization and disinfection principles relevant to laparoscopic and other medical instruments. It distinguishes between sterilization, disinfection, and asepsis, emphasizing the critical role of thorough pre-cleaning in ensuring effective microbial elimination. Key factors influencing sterilization efficacy—such as organic load, device design, biofilm presence, and exposure conditions—are discussed in detail. The chapter reviews various classes of chemical agents, including alcohols, aldehydes, halogens, oxidizing agents, phenolics, and quaternary ammonium compounds, highlighting their mechanisms of action, applications, advantages, and limitations. Special focus is given to high-level disinfectants like glutaraldehyde and ortho-phthalaldehyde for heat-sensitive instruments. Limitations of liquid chemical sterilants, particularly in achieving sterility assurance comparable to thermal methods, are also addressed. An evidence-based approach to selecting appropriate disinfectants is emphasized to ensure patient safety and optimal infection control in healthcare settings.
