Laparoscopic bariatric surgery has evolved significantly with advancements in specialized instruments that enhance safety, precision, and efficiency in managing morbidly obese patients. This chapter outlines the essential instruments used in bariatric procedures, emphasizing their design adaptations for increased body habitus and technical complexity. Key components include high-capacity operating tables, access devices such as Veress needles, optical trocars, and extra-long trocars, as well as advanced laparoscopes providing improved visualization. Retraction systems, including liver retractors and innovative suspension techniques, are crucial for optimal exposure. A wide range of hand instruments—graspers, scissors, suturing devices, and knot pushers—along with specialized tools like the Goldfinger dissector, facilitate precise dissection and tissue handling. Energy devices and advanced stapling technologies further contribute to haemostasis and secure anastomoses. Understanding the appropriate selection and application of these instruments is vital for overcoming technical challenges and optimizing surgical outcomes in bariatric surgery.
