Prospects for CT-FFR Research and Application

This chapter explores the advancements and clinical prospects of coronary computed tomography angiography-derived fractional flow reserve (CT-FFR) in managing coronary artery disease (CAD). It highlights innovations such as fully automated CT-FFR software, which enhances diagnostic accuracy and reduces analysis time, and discusses integrated scoring systems (e.g., functional SYNTAX, CAD-RADS, Duke scores) for improved risk stratification and decision-making. Large-scale trials, including FORECAST and China CT-FFR studies, demonstrate CT-FFR’s potential to reduce invasive angiography use while maintaining prognostic reliability. Despite progress, challenges like computational variability and the need for prospective randomized evidence persist. Future research aims to refine CT-FFR’s integration with anatomical and functional assessments to optimize CAD management globally.
