A 32-year-old primigravida woman was admitted for rupture of the membrane. She did not have regular antenatal follow-up. Because of fetal distress, the obstetrician decided on an emergency caesarean section. A baby with an estimated maturity of 36 weeks was delivered. The birth weight of the baby was 2.85 kg. He was flaccid and cyanotic at birth and required immediate resuscitation. The baby was intubated at 1 min of life and was transferred to the neonatal intensive care unit. His Apgar score was four at 1 min, seven at 5, and eight at 10 min. If attempts to ventilate this baby was ultimately unsuccessful, what might be the underlying causes?
