Isabella is an 11-year-old girl who has been healthy all along. She had a history of a febrile upper respiratory infection 2 weeks before admission, and she recovered with a residual cough. She complained of puffy eyelids and abdominal fullness for 2 days. Hence, she was admitted to the emergency department. She had a chest X-ray taken in the emergency room. The duty house officer assessed her and informed the paediatric resident as she was not sure about the chest X-ray findings (Fig. 10.1a). Isabella looked puffy and gained 2 kg in the past 1 week. Her blood pressure was 90/60 mmHg with a heart rate of 110 bpm. Her oxygen saturation was 92% in room air.
