Neurorehabilitation is a multidisciplinary, goal-oriented intervention aimed at enhancing recovery and quality of life in patients with neurological impairments. This chapter critically explores ongoing controversies in neurorehabilitation, focusing on the timing and intensity of interventions, physical versus cognitive prioritisation, outcome assessment metrics, and management strategies in severe brain injury. While early rehabilitation is associated with improved neuroplasticity and functional outcomes, concerns remain regarding safety and individual readiness. Similarly, the superiority of high-intensity protocols is debated, with evidence suggesting benefits while also cautioning against overexertion. The chapter also addresses disparities in access and implementation between high- and low-resource settings, the appropriateness of intracranial pressure (ICP)–focused care in acute traumatic brain injury (TBI), and the limitations of current outcome measurement tools. A patient-centred, multidisciplinary, and phased strategy is advocated to enhance functional independence and long-term recovery.
