Radiologic Evaluation of Lumbar Spinal Stenosis

Lumbar spinal stenosis causes compression of the nerve roots leading to symptoms such as pain, claudication, and weakness. Plain radiographs, computerised tomography (CT), and magnetic resonance imaging (MRI) are the main imaging modalities utilised in the assessment of spinal stenosis. Plain radiographs help in the assessment of spondylolisthesis, instability, disc height, and sagittal and coronal alignment. CT helps to determine if the spinal stenosis is bony or due to soft tissues. It also helps to identify facet arthropathy, calcified disc, pars defect, ossified ligamentum flavum, and osteophytes. MRI is the most important imaging tool in evaluation of spinal stenosis as it provides optimal visualisation of soft tissues. It helps in the identification of disc herniation, ligamentum flavum hypertrophy, facet cyst and effusion, and abutment/compression of dural sac and nerve roots. Stenosis can involve the central canal, lateral recess, neural foramen, or a combination of these. Localisation and grading of lumbar spinal stenosis along with clinical correlation are vital for treatment success.
