Application of Pedicled Perforator Flap in the Reconstruction of Oral and Maxillofacial: Head and Neck Defects

In 1989, Japanese scholars Koshima et al. reported for the first time that the free perforator flap was used to reconstruct tissue defects, which broke the constraint of the traditional idea that “deep fascia is the basis of the survival of skin flaps.” Perforator flaps have been developed and popularized rapidly over the past 20 years because of their advantages of flexible design, no sacrifice of important nerves or blood vessels in the donor site, constant blood supply, long vascular pedicle, direct closure of the donor site, and aesthetics of the recipient site, etc. Especially in recent years, many scholars, based on the core idea of minimally invasive and artistic perforator flaps, have been innovating the operating methods so that various types of perforator flaps, such as flow-through perforator flap, microdissected thin perforator flap, conjoined perforator flap, polyflaps perforator flap, and chimeric perforator flap have emerged, which allowed the surgeons more flexible in their operations in the reconstruction field.
