The concept of perforator flap was first proposed by Koshima and Soeda in 1989. A perforator flap refers to a kind of small axial-pattern flap, to which blood is only supplied by the skin perforator vessels with small calibers (the caliber ≥0.5 mm after the vessel passing through deep fascia), including skin and subcutaneous tissues. Its axial vessel is a perforator (perforator artery and vein). A perforator vessel refers to a nutrient vessel originating from the source vessel and supplying blood to the subcutaneous tissues and skin through the deep fascia. With the development of microsurgical instruments and clinical techniques, perforator flaps have been widely used in microsurgical reconstruction by virtue of their advantages such as less trauma and functional impact on the donor site, flexible operation, personalized design, and so forth.
