Anatomical Basis of Perforator Flaps

Perforator flap was firstly proposed by Koshima first 1989. In a narrow sense, it refers to axial-pattern the flaps for whose blood supply is only provided by skin perforator vessels with a small diameter (the diameter after passing through deep fascia is 0.5 mm or bigger) and containing skin and subcutaneous tissue. Its axial blood vessel is perforator vessel, namely, perforator artery and vein. Perforator flaps generally don’t contain deep fascia in terms of structure. With the continuous promotion and application of perforator flaps in clinical practice, their concept has been expanded to all flaps supported by perforator vessels in a broad sense. As long as a flap has a direct blood-supply pedicle which is a perforator artery and a perforator vessel is obtained from surgical dissection and separation, with or without the superior source artery depending on actual clinical needs, it should be referred to as a perforator flap clinically. As a limited and strict concept anatomically, a perforator flap refers to the flaps supported by a short segment of perforator vessel giving off from the source artery and running to the shallow layer.
