Graves’ disease often is associated with a variety of other autoimmune diseases as well as several cardiac and neurologic complications. Of the neurologic manifestations, thyrotoxic periodic palsy is a rare channelopathy with dramatic presentation. It is more common in young Chinese males with GD where treatment with ATD and β-blocker is all that is necessary to prevent further episode. Here, we discuss the case of a 38-year-old male with Graves toxicosis presented with abrupt flaccid quadriparesis. He was found to have severe hypokalemia that was complicated by heart block precipitated by β-blocker therapy. It highlights the challenges that we faced and dealt with while evaluating and managing such a patient.
