This is the sad story of a young lady who was evaluated for an adrenal incidentaloma found on USG of abdomen. Initially, she was found negative for pulmonary TB when investigated based on fever, weight loss, and dry cough. Following admission into our unit, she was reported to have fluctuating HTN with target organ damage and admitted of having a typical Pheo spell. Interestingly, she also had a moonface, hyperpigmentation, prominent proximal myopathy, and hypokalemia. Biochemical and radiologic investigations were in favor of an ACTH-producing pheochromocytoma, where the immunosuppressed state favored opportunistic infection like TB that proved positive on retesting. She was discharged on anti-TB and antihypertensive medication with a future plan of surgery; unfortunately, she died soon after following a Pheo spell resulting in pulmonary edema.
