This is the story of a middle-aged man who was evaluated for uncontrolled HTN and debilitating proximal myopathy of recent onset. He had a H/O surgery for invasive olfactory neuroblastoma (ONB) several years earlier; he took no further treatment post-surgery. His biochemical investigations confirmed the presence of ACTH-dependent Cushing syndrome, though a variable report of ACTH created some confusion initially. Finally, the source was localized following endo-nasal surgery for recurrent olfactory neuroblastoma that stained positive for ACTH. It is one of the few reported cases of ectopic Cushing syndrome due to the paraneoplastic expression of ACTH by ONB.
