This case describes the unique co-occurrence of a pituitary microadenoma and an adrenal nodule in a young female with Cushing syndrome. This young lady with typical stigmata of Cushing syndrome was biochemically confirmed to have an ACTH-dependent disease. However, the presence of an adrenal nodule led to a diagnostic dilemma. The resected pituitary tumor showed Crooke’s hyaline change and had positive ACTH immunostaining. Finally, significant clinical and biochemical improvement along with regression of the adrenal nodule following pituitary adenomectomy proved the ultimate diagnosis of Cushing disease.
