The development of living donor liver transplantation (LDLT) has primarily been driven by the significant disparity between the number of patients awaiting transplants and the limited availability of deceased donor organs. Despite the complex ethical considerations involved, LDLT remains a technically intricate procedure that should only be undertaken when the potential benefit to the recipient justifies the risk to the donor. The fundamental objectives of LDLT are to ensure minimal morbidity and nearly zero mortality for the donor while delivering a meaningful survival advantage to the recipient. Achieving these outcomes heavily depends on thorough donor evaluation, which is a critical component of a successful LDLT program. This chapter outlines the criteria for donor selection and the evaluation process and offers a brief overview of the surgical procedure involved in living liver donation.
