The ovarian neoplasms are a diverse group that reflect the various histologic elements found in the organ. The tumors can be broadly categorized into three groups having an epithelial cell, germ cell, and stromal cell origin. Some of the ovarian neoplasms contain a mixture of different cell elements and are poorly differentiated, and are difficult to categorize into the three groups. The ovary is a common site of metastases from various malignant tumors, including carcinoma of the stomach and carcinoma of the colon. The overlap of imaging appearances between metastatic ovarian tumors and primary ovarian tumors is substantial, and a confident distinction between the two is very difficult on the basis of imaging findings. Primary fallopian tube carcinoma is an unusual gynecologic malignancy that is rarely diagnosed correctly before surgery due to difficulty in assessing the imaging findings. This chapter describes imaging features of various kinds of metastatic ovarian tumors, rare ovarian tumors those are difficult to categorize into the usual categories, and primary tumors of fallopian tubes and broad ligaments.
