Endometrial carcinoma (EC) is the most prevalent gynecologic cancer globally, with increasing incidence. Early stage detection is common due to abnormal uterine bleeding in most patients, leading to a high overall 5-year survival rate. The 2023 FIGO staging system incorporates histological types, lymphovascular space invasion (LVSI), and molecular classifications for the staging. Imaging modalities such as transvaginal ultrasound, CT, MRI, and FDG PET-CT play crucial roles in diagnosis and treatment planning. MRI, with its superior ability to evaluate myometrial and cervical stromal invasion, is recommended for preoperative assessment. The incorporation of radiomics and emerging technologies like PET-MR shows promise for improved diagnostic accuracy. This chapter highlights the latest updates in FIGO staging and the significant role of various imaging modalities in the management of EC.
