Patient is a 64-year-old male. He suddenly started to experience severe right waist and gluteal pain after long time sitting about half a year ago and prevented him from standing up with relief after rest. This had become episodic, usually occurred after long time standing or walking and could be incited by coughing or sneezing and radiated to the right lower extremity. Patient took pain killer and ointment himself. He was also treated with pain killer, nerve nutritional medication, and acupuncture at the local community hospital without any relief.
