Pre-transplant immunological workup of potential transplant recipients to determine and stratify risk has been a constantly evolving field. The process uses a combination of platforms and tests usually combining a cell based and solid phase assay. The most commonly used platforms are the complement dependent cytotoxicity (CDC), flowcytometric, and luminex based assays in increasing order of sensitivity. The main goal is to identify donor specific antibodies (DSAs) that can be detrimental to the graft. The flowcytometric crossmatch and the CDC based crossmatch use donor lymphocytes, while the single antigen bead (SAB) assay on the luminex crossmatch is a virtual crossmatch. This chapter describes four different scenarios where patients with DSA on the SAB assay are evaluated and managed.
