This case report involves a 25-year-old Indian male with stage V chronic kidney disease (CKD), who is undergoing evaluation for an ABO compatible living-related kidney transplant. The donor is a 25-year-old paternal cousin. The donor-recipient have a 3/6 mismatch. The recipient’s immunological workup shows relatively low sensitization to HLA which is related to prior blood transfusion. This raises interesting questions regarding the implications for the immunological risk of recipient donor pair.
