Hematopoietic stem cell transplantation (HSCT) is a curative procedure for many benign and malignant blood disorders. The survival outcomes have improved dramatically over the years from the historical failures of transplants performed initially for advanced leukemias and immunodeficiencies. The entire procedure is strategically planned to establish immune homeostasis between the donor and recipient hematopoietic systems after considering disease biology, age, and donor and recipient Human Leukocyte Antigen (HLA) disparity. The type of conditioning chemotherapy, graft-versus-host disease (GVHD) prophylaxis, and antimicrobial prophylaxis vary between autologous and allogeneic transplants. We describe the course of HSCT performed for two representative cases of Acute Myeloid Leukemia (AML) at our center to highlight the complexities in decision-making over time and its influence on outcomes.
