Fever of unknown origin is sometimes due to various connective tissue diseases/non-infectious inflammatory diseases. The proportion of this group of diseases in FUO has increased in recent years, accounting for about 20%–30%. Common causes include rheumatoid arthritis (RA), systemic lupus erythematosus (SLE), adult-onset Still’s disease (AOSD), vasculitis, dermatomyositis/polymyositis (DM/PM), mixed connective tissue disease, etc. Due to the development of sensitive laboratory diagnostic tests (including antinuclear and anti-DNA markers), rheumatic/noninfectious inflammatory are easier to diagnose. The diagnosis is now usually made within 3 weeks. However, the trend of aging results in increasing of the incidence of rare diseases, such as dermatomyositis/polymyositis and temporal arteritis in elderly patients.
