Multiligament knee injury constitutes a tear of two or more than four ligaments of the knee. The words knee dislocation and multiligament knee injuries are frequently used synonymously. Knee dislocations frequently lead to multiligament knee injuries; however, certain multiligament knee injuries do not include knee dislocations. The entity is challenging due to both the diagnostic and reconstructive procedures, as well as the subsequent rehabilitation program following the surgery. Concurrent meniscal, chondral, and nerve injuries frequently occur in multiligament injuries, requiring a thorough assessment. Current literature favors surgical intervention for multiligament knee injuries, with reconstructions advised due to the higher failure rates associated with ligament repair. It is recommended to reconstruct all ruptured ligaments in a single stage, if feasible, to facilitate early mobility and prevent joint stiffness. In the setting of multiligament knee reconstructions, some critical technical considerations include graft selection, the sequencing of ligament reconstruction, tunnel positioning and orientation to prevent tunnel interference, and the order of graft tensioning. Postoperative recovery following a multiligament knee repair often necessitates 9–12 months of therapy before resuming full activity.
