Current Trends and Innovative Treatment Approach to Knee Injury in Sports

This chapter explores recent advancements in knee arthroscopy, focusing on the treatment of ligament, meniscus, and articular cartilage injuries. Significant strides in understanding knee anatomy and biomechanics, alongside innovations in instrumentation and implants, have transformed the management of knee injuries. The field has shifted towards preserving intra- articular structures, such as the anterior cruciate ligament (ACL) and menisci, through advanced repair and reconstruction techniques. ACL injuries remain a focal point, with techniques like ACL repair, remnant-preserving ACL reconstruction, and dynamic stabilisation showing promise for faster recovery and improved outcomes. The emergence of regenerative adjuncts and hormone-based prevention strategies further enhances ACL injury management. Meniscal preservation is another key area, with augmentation strategies such as platelet-rich plasma (PRP) and meniscal scaffolds showing potential for improved repair success rates. Meniscal allograft transplantation and scaffold implantation offer viable alternatives for meniscus loss, while new technologies like retrograde anchor fixation simplify complex repairs. For articular cartilage injuries, regenerative approaches, including cell-based therapies and orthobiologics, are gaining traction. Surgical techniques like osteochondral autograft transplantation, autologous chondrocyte implantation, and scaffold-based treatments are being explored for cartilage regeneration, showing promise for long-term joint health. These innovations are transforming knee arthroscopy, offering new hope for sports-related injuries and joint preservation.
