Does a lower hemoglobin threshold for transfusion improve outcomes in patients with septic shock? The TRISS trial aimed to determine whether a lower hemoglobin threshold for red blood cell transfusion could improve outcomes in patients with septic shock. This multicenter, randomized controlled trial enrolled over 1000 critically ill patients with septic shock and randomized them to receive either a lower hemoglobin threshold for transfusion (7 g/dL) or a higher threshold (9 g/dL). The study demonstrated that a lower hemoglobin threshold (7 g/dL) did not result in higher mortality compared to the higher threshold (9 g/dL). Furthermore, the lower threshold group showed comparable or better outcomes in terms of organ failure and mechanical ventilation. These findings suggested that a restrictive transfusion strategy (7 g/dL threshold) is safe and may be preferable in septic shock patients.
