Coronary Angioplasty Versus Fibrinolytic Therapy in Acute Myocardial Infarction: The DANAMI-2 Trial

Which is better in acute myocardial infarction (MI)—Percutaneous Coronary Intervention (PCI) or fibrinolytic therapy? The DANAMI II trial PCI with fibrinolytic therapy in patients with acute MI presenting within 12 hours of symptom onset. Patients were randomized to receive either PCI or fibrinolysis. The study found that PCI was superior to fibrinolytic therapy in reducing the risk of death, recurrent myocardial infarction, and the need for repeat revascularization. Patients who underwent PCI also had better long-term survival outcomes. The results from this trial reinforced the benefit of PCI over fibrinolysis in STEMI patients, especially those presenting early, and provided strong evidence for early angioplasty as the preferred strategy in this patient group.
