Are shorter antibiotic courses adequate for ventilator-associated pneumonia (VAP)? This randomized controlled trial compared the duration of antibiotic therapy (8 days vs. 15 days) for VAP. A total of 1015 patients with VAP were randomized to receive either 8 or 15 days of antibiotics. The study found no significant difference in clinical outcomes, including mortality, between the two groups. However, the shorter duration of therapy (8 days) led to reduced antibiotic use, lower costs, and fewer adverse effects related to prolonged antibiotic treatment. The trial provided strong evidence that shorter courses of antibiotics are equally effective in treating VAP, supporting a shift toward more conservative antibiotic strategies to reduce resistance and complications.
