Can smaller breaths save lives in ARDS? The ARDSNet trial compared two different tidal volumes in patients with acute respiratory distress syndrome (ARDS): 6 ml/kg of predicted body weight vs. 12 ml/kg. This large, multicenter, randomized trial involved 861 patients and found that using a lower tidal volume (6 ml/kg) significantly reduced 28-day mortality compared to the standard higher tidal volume (12 ml/kg). The study demonstrated that lower tidal volume ventilation was not only safer but also led to more favorable clinical outcomes, including improved survival and more ventilator-free days with a low tidal volume, lung-protective ventilation strategy. This landmark trial established low tidal volume ventilation as the standard of care for patients with ARDS.
