Alternatives to Antibiotics

Since their discovery in the twentieth century, antibiotics have been vital in reducing deaths from bacterial infections. However, their overuse and misuse have accelerated the crisis of antimicrobial resistance (AMR), rendering many treatments ineffective and posing a significant threat to global health. The slow pace of new antibiotic discovery—due to scientific, financial, and regulatory barriers—necessitates urgent exploration of alternatives. Research is ongoing on several options to antibiotics, including bacteriophages, antimicrobial peptides, probiotics, immunotherapies, and novel synthetic compounds. Phage therapy offers targeted action against resistant pathogens, while probiotics and microbiome modulation restore microbial balance and enhance host defences. Monoclonal antibodies and immunotherapies provide precise immune-based interventions, whereas novel synthetic agents and antibody-antibiotic conjugates aim to bypass conventional resistance mechanisms. Other approaches, including photodynamic therapy, gene-editing tools like CRISPR-Cas, and artificial intelligence-driven drug discovery, present innovative possibilities. In the veterinary sector, alternatives such as vaccines, probiotics, plant-derived products, and ethnoveterinary practices are gaining traction to reduce antibiotic use in livestock and safeguard One Health. While no single solution can entirely replace antibiotics, integrating these alternatives with stewardship, biosecurity, and improved animal husbandry can significantly reduce reliance on traditional drugs. Together, these innovations represent critical pathways to contain AMR and secure sustainable health systems.
